Prior to enrolling your child at Maternity B.V.M. Elementary
School you will be required to provide the following information
for our records. If you have any questions about how to obtain

KINDERGARTEN:
1. Original Birth Certificate
2. Baptismal Certificate (if Catholic)
3. Current Physical Examination & Immunization Records
4. Current Dental Exam

GRADES 1-8:
1. Transfer Records
2. Latest Report Card
3. Original Birth Certificate
4. Baptismal Certificate (if Catholic)
5. Health Form *
6. Current Physical Examination 5th Grade
7. 5th Grade Students Need Three(3) doses of Hepatitis B    
Vaccine
8. 2nd and 6th Grade Students must have a Dental Examination
